Information collected

The form collects the name, email address, enquiry category and message supplied by the sender. A technical anti-spam field may also be processed.

Purpose and legal basis

The information is used only to review and respond to professional enquiries and to maintain necessary correspondence. Processing is based on the sender’s request and, where applicable, legitimate interests in managing professional communications.

Services used

Netlify processes and stores form submissions as the website hosting and form-service provider. If email notifications are enabled, the site owner’s email provider may also process a copy of the submission. No contact-form information is sold or used for advertising.

Retention

Messages are retained only for as long as reasonably necessary to respond and manage the related professional correspondence. Routine enquiries should normally be deleted within 12 months after the last relevant communication, unless a longer period is required for an ongoing collaboration, legal obligation or legitimate record-keeping purpose.

Sensitive information

The form is not intended for medical records, genetic test reports, symptoms, identifying patient information or detailed family histories. Please do not submit this material.
